Anytime you can see an explosion, you run the chance of getting hit by debris.

Also, you'd think the camera man would use not only a zoom lens to get in closer, but be back further. That way he'd still have the same view (of an explosion?? why not pan out?).

The camera did zoom out. And I bet the cameraman hadn't zoomed out all the way too. He was at a safe distance, supposedly, but a telephoto lens can only go so far.
