You'd figure there would be some sort of interlocking device alerting the engineer to a door that has an obstruction.
A $3.00 switch would do it.
Or someone like a conductor on the platform ensuring everyone was "ALL ABOARD!" before allowing the engineer to leave the station.

I thought it was a law to have those devices installed, every one that I have been on, no matter what country has had those installed.
